Channel 4’s Surgery Secrets of the Super Rich follows Joyce as she makes a deeply personal decision. After years of exploring non-surgical treatments, she’s finally decided to undergo facial rejuvenation surgery with Dr Tim J Janssen.
The documentary goes beyond just a before-and-after result, it explores Joyce’s consultation, preparation, the operation itself and the three-month recovery process.

Joyce’s story offers an honest unfiltered look at the emotional and physical realities of recovery. Swelling, bruising and temporary changes in appearance are portrayed as part of the healing process rather than hidden.
A 60-year-old grandmother and mother of two, Joyce came to Dr Janssen seeking comprehensive facial rejuvenation. Her goal was to maintain her recognizable features and expressions.
Despite years of non-invasive treatments like HA filler and thread lifts Joyce achieved only marginal results. Multiple energy device treatments further complicated matters by causing extensive deep scarring and filler fatigue. The filler led to a stretched heavy puffy appearance due to oedema, a common side effect of repeated water-attracting (hydrophilic) filler injections over time. Despite the potential impact on recovery including prolonged swelling Joyce trusted Dr Janssen with her surgery, Joyce trusted Dr Janssen with her surgery.
Joyce’s priorities
Every face ages differently. During Joyce’s consultation, Dr Janssen assesses the position of the deeper facial tissues, skin quality, facial volume, brow position, jawline and neck anatomy. Multiple previously performed non-surgical interventions have caused accelerated descent of the tissues, chronic edema and extensive deep tissue scarring.
For natural, long-lasting results, Joyce needed more than a lift of a single area. A deep plane facelift is most effective for the lower face and neck while an endoscopic facelift focuses on the midface and brow. Therefore Joyce’s treatment combined endoscopic and open deep plane elements, allowing Dr Janssen to choose the best approach to achieve harmonious results. In our practice, this technique is referred to as Hybrid Endoscopic face and neck lift. The Hybrid HI LiftTM is one of the few techniques providing significant lift throughout the face’s proportions, including traditionally difficult-to-treat areas like the midface..

Joyce’s experience demonstrates why facelift recovery requires preparation, patience and trust. While social recovery is often measured in weeks, biological healing may continue for much longer.
In the episode, we saw Joyce at six weeks post-surgery. Swelling, especially around the eyes cheeks and jawline, can still be noticeable and alter facial features. Scars may also remain active and visible and their appearance might fluctuate rather than improving steadily daily.
Similar to Joyce’s previous experiences with dermal fillers and energy-device treatments like radiofrequency micro-needling, the extent of the surgery and individual healing characteristics significantly impact recovery. Careful pre-operative preparation and planning have helped Joyce maintain a positive outlook throughout her journey.
The final result of facelift surgery should never be judged during the earliest stages of healing.
